In addition to the forms above certified leaders are required to undergo both a Pennsylvania State Child Abuse Clearance and Criminal Background Check. Adobe Acrobat Reader and PDF files: To download forms and files from this site, and many other internet sites, you will need to download and install Adobe Acrobat Reader. You will download a file named rs405.eng.exe or something similar. Save it to a folder on your hard disk. Click on it and it will self-extract and install.

When you download a form or other document, the Acrobat Reader will open up. You can print forms, you can save form to disk as a PDF (portable document format) file, but you cannot edit forms. You can copy from forms and paste into another document. You can open saved forms by clicking on them or by opening them in the Acrobat Reader.
